Kuchisake-onna (口裂け女, Slit-Mouthed Woman) is a powerful illusion incorporating the use of Kirigakure's signature mist as well as the Magen: Kyōka Suigetsu. It is recognised by Heiwa as his most horrific and twisted illusion due to the traumatic side-effects victims whom it was face.

By forming a thick mist full with the user's chakra, said user can then apply the Yin Release to the technique, casting an illusion which the mist dissipates but the user's presence is also erased. A slit-mouthed ghostly woman also appears from behind the victim, whom soon becomes paralyzed. The user is then mutilated slowly. Whilst feeling the entire pain of having their limbs removed from them, the victims are incapable of falling asleep nor are they capable of death. Their very own flesh is then eaten before their very own eyes, before the woman begins to bulge out the tongue of the target, and then the eyes. After that, the target becomes incapacitated. Releasing the technique through merely stopping one's chakra flow is fruitless, as the illusion is cast through their surroundings.

The technique in it's own right is incredibly powerful, and thus it causes terrible side-effects to it's victims. Nightmares of their own limbs ripped out and eaten plagues them until their death, unless they attend therapy sessions. The trauma continues to resurface whenever a masked person, appears, triggering their horrific events. Even the strong-willed succumb to the horrors of the illusion.
